Online Dues Assignment Card

Subject to the limitations imposed by the State Laws. I hereby designate as my collective bargaining representative, Law Enforcement Employees Benevolent Association, (L.E.E.B.A.) and authorize and direct my employer to deduct and pay to it from my wages such amounts for my initiation fees and monthly dues as from time to time are certified by said labor organization to be due and owing to it as periodic dues and initiation fees uniformly required by L.E.E.B.A., as a condition of acquiring/maintaining membership therein. Such amounts are to be deducted each month and to be forwarded to the Secretary-Treasurer of L.E.E.B.A.

DCAS Police Decertification and Authorization Form

This decertification and authorization form will be submitted to the New York City Office of Collective Bargaining. Under their rules, if 30% or more of DCAS Police sign and submit these forms, an election will be held to determine what organization will represent DCAS Police as the exclusive bargaining representative.
